3. Photocopy of Certificate of Eligibility/Report of Rating;
4. Photocopy of valid and renewed PRC License/ID, if applicable;
5. Certificate of Employment, Contract of Service, or Duly signed Service Record;
6. Certificate of Trainings/Seminars, if applicable;
7. Photocopy of scholastic/academic record, e.g. Transcript of Records (TOR),
including graduate and post-graduate with certification;
8. Checklist of Requirements and Omnibus Sworn Statement on the Certification
on the Authenticity and Veracity (CAV) of the documents submitted and Data
Privacy Consent Form Pursuant to RA No. 10173 (Data Privacy Act of 2012),
notarized by authorized Official; and
9. Other pertinent documents.
