Monitoring clients’ progress and recommending changes to treatment plans as needed
Coordinating services with other social workers, physicians, nurses, therapists, psychiatrists, and other healthcare professionals
Conducting interviews with prospective foster parents or adoptive parents
Coordinating service delivery with other agencies or community groups such as hospitals, law enforcement agencies, schools, and businesses
Facilitating groups for clients who are coping with loss or grief, including children who have experienced loss of a parent or other significant family member
Developing individualized plans for clients with chronic mental illness or physical disabilities who need assistance with daily activities such as bathing, dressing, eating, or shopping
Participating in court hearings and providing testimony regarding a client’s progress or need for continued services
Preparing reports on clients’ progress or lack thereof
Identifying client needs and referring them to appropriate resources or agencies for assistance
